Commission Chart help

I've never made a commission chart before and was wondering if anyone had advice on making one or templates I could look at?
I'd be offering colored sketches, flats, and shaded pieces, and possibly an additional painterly-like style. Ideally I'd like 2 examples for each but would that be too much to fit onto a chart?

Any advice appreciated. I'm used to just writing out all my commission info but I feel that a chart could help condense it.

It's usually best to keep to one example for each thing, and you can leave off sketches or less detailed things if you wish. Try to make it professional, not too cluttered looking.
